Pronunciation rules

As an English and public speaking professor, I notice things and wish people would get them right.  Now my curmudgeonly part comes out.

Why do people say, "small, little" whatever? 

Why do people say "often" with the "t"?  It's like soften and listen, people.  You're being overcorrect.

Why do people say "heighth" with a "th" sound at the end?

Why do people say "The letter was sent to Mary and I" or similar constructions involving prepositions? 

Add to the list, oh fellow purists!
